Description

Lactose and fructose are normally absorbed in the small intestine. If there is malabsorption, they reach the large intestine intact, where they are metabolized by the microbiota. In this process hydrogen and/or methane gases are generated, which are absorbed, pass into the bloodstream and are transported by the blood to the lungs, where they are eliminated with the exhaled air. Thus, an increase in the concentration of these gases in the breath after administration of lactose and/or fructose indicates that they have not been absorbed correctly and, therefore, there may be malabsorption or intolerance to lactose and/or fructose.

In the case of lactitol, in healthy individuals it reaches the intact colon, where it is fermented by the microbiota. However, in individuals with small intestinal bacterial overgrowth, it is metabolized in the small intestine, giving rise to hydrogen and/or methane gases that are detected in the breath.
